I believe I have the issue noted in previous posts noted by Pandora Support: "For future reference, when you submit the survey that puts you over the 50 point mark, you will get automatically redirected to a new URL where you have to fill out a simple form with your email address to request the reward." However, it seems i was unable to be redirected to that form to get reward email, and appears to have lost the 50 points, now showing 0 survey points in survey emails with no reward email.
