iOS: Unable to remove songs

No matter how many times i click remove  from songs or my collections it just keeps re adding the song. I click the check mark it re checks it same with the 3 little dots. If that song plays I cant skip it or anything.

To clarify, is this happening when you try to remove songs from your Collection? Or are you trying to delete songs from a playlist?

Please provide the name of the songs you're trying to remove.

If this happening on a playlist, please also provide the name of the playlist(s) this is happening.
